Transfer from the care of the Children's Department to the Adult Department
Once an individual reaches the age of 18, any help they need in terms of social care will come from the Department of Adults, Health and Wellbeing. If an individual received support as a child then discussions and an assessment will be required as soon as possible in order to establish if there are care and support needs that should be met by the Adult Department.
Transfer
When the service user is between 16 and 18 years old, plans will be made with the young person and their families, to prepare them to move to adult services. This is called transfer.
We know that any change will be difficult but we will do our best to ensure a smooth transition and we will be here to support you every step of the way.
During the transfer it will be necessary to discuss:
- Health and social care
- Mental health
- Education
- Financial benefits for the young person and their family
- Work
- Accommodation and support
- Well-being
Once we have gathered the information about these areas we can better assist in preparing the child for the world as an adult and the assessment will consider
- the requirements of the service user
- what is important to the service user
- what the service user wants to achieve in life
- what kind of care or support can benefit the service user
- what other help is available?
We can then move on to deliver the best care, advice or support for you.
As you go through the process of transitioning to adult services, you will have a specific worker who will support you in organizing plans and we will commit to helping you have the best possible future.
